HISTORY:

A 67-year-old woman with history of hydronephrosis and urinary retention, chronic kidney disease. No recent fevers or new LUTS, has burning at the urethra with her foley catheter in place. Past history is significant for diabetes mellitus, hyperkalemia, hypertension, stroke, and UPJ (ureteropelvic junction) obstruction. Cystoscopy revealed erythema, inflammation, and a few areas of papillary changes, also sloughing mucosa in the dome/posterior wall of bladder.

What is the most likely diagnosis?

  1. Urothelial carcinoma, nested/microcystic variant
  2. Clear cell adenocarcinoma
  3. Nephrogenic adenoma/metaplasia
  4. Prostatic adenocarcinoma of bladder
