HISTORY:

A female patient in her 40s presented with dull left-sided flank pain which was associated with gross hematuria.

CT scan revealed a X cm solid renal mass in the left kidney with irregular enhancement raising suspicion for renal cell carcinoma.

Clinicoradiological diagnosis of RCC was made and left radical nephrectomy was performed.

The resected kidney weighed 34.3-gram and measured 5.8 x 4.1 x 3.2 cm. Gross examination revealed a a 2.1 x 2.9 x 2.1 cm slightly irregular, brown-orange and hemorrhagic mass. The mass is 90% necrotic and, abuts the renal parenchymal margin and capsule without perforation. The remaining renal parenchyma has a 0.7 cm thick cortex and 1.2 cm length of medulla.

What is the most likely diagnosis?

Clear Renal Cell Carcinoma with:

  1. Teratoma
  2. Osseous metaplasia
  3. Dystrophic Calcification
  4. Osteosarcoma
