HISTORY:

A Female in her 60s presented with right side flank pain, gross hematuria.

Right renal cell carcinoma was diagnosed and the patient underwent Radical Nephrectomy and adrenalectomy.

Grossing revealed solitary, irregularly marginated lesion within the mid portion and lower pole of the kidney measuring 6.7 cm, prolapsing into the renal vein, involving the perirenal fat.

It also revealed a smooth-walled cortical cyst posteriorly (1.4 cm), and a punctate gray medullary lesion within the superior pole (0.4 cm) as shown in the image 1A,1B,1C.
